To aid the registration process, please provide us with your NHS number if possible. If you do not know this, you can find it online here.
If you wish to register to use our online services, you will need to complete an online services registration form and provide us with proof of ID. Please take a moment to complete this short form to verify your contact details. It is especially important that we hold your current mobile telephone number on your record, and that you let us know if this number changes.
We always try to provide the best service, but there may be times when you feel your expectations have not been met. If so, we hope you will let us know, allow us to investigate and, if necessary, put right any issues or mistakes that have been made.
To make a complaint or to provide any complaints or suggestions, please telephone, email, or write to our Patient Service Manager, Jocelyn Holland.
If your complaint involves a Doctor, you may prefer to approach that Doctor directly. We feel it important to deal with complaints swiftly, so you will normally receive an initial response within two working days, pending a full investigation.
We will try to address your concerns fully, provide you with an explanation and discuss any action that might be needed. Please note we must respect our duty of confidentiality to patients, therefore patient’s consent will be necessary where if patients do not make a complaint in person.
We hope you will feel satisfied we have dealt with the matter thoroughly. However, if this is not so and you wish to continue with your complaint, we will direct you to the appropriate authorities who will be able to help you.

Patients are advised that authorised clinicians that provide them with care outside of our surgery will have access to certain patient information via the ‘Summary Care Record’.

The PALS or complaints manager at the NHS organisation you are complaining about can provide advice, including about local independent complaints advocacy services. Your local primary care trust can also advise if you have a complaint about a primary care practitioner.
The Independent Complaints Advocacy Service (ICAS) provides advice and support to people who want to complain about the NHS. Details are at www.nhscomplaintsadvocacy.org, or telephone 0300 330 5454

The NHS is a large and complex organisation, with over a million staff providing a broad range of services across thousands of locations.
You may at times feel bewildered or concerned when using the NHS. The Patient Advice and Liaison Service, known as PALS, was introduced to ensure the NHS listens to patients, their relatives, carers and friends, and answers their questions and resolves their concerns as quickly as possible.
PALS also helps the NHS to improve services by listening to what matters to patients and their loved ones and making changes, when appropriate.
