If you reside within our practice area you can register with us. You can print off a new patient registration form, complete it and either send it to us in the post or bring it to reception when you visit the surgery.
Please note that we will not be able to accept your registration form without your NHS number completed – you can get your NHS number from your NHS card or by telephoning your present surgery.
When you register at the practice you will also be asked to complete a new patient medical questionnaire. This is because it can take some time for us to receive your medical records from your previous practice. Printable versions of these forms are available from the links below. Please print and complete BOTH forms, and return them to us either in person or by post.
Patients are advised that authorised clinicians that provide them with care outside of our surgery will have access to certain patient information via the ‘Summary Care Record’.
